- Pain compatible with angina pectoris AND
- ECG which does not meet the criteria for STEMI or STEMI equivalent AND
- Rise or fall of cardiac biomarker values with at least one value above the 99th percentile, possibly by point of care measurement AND/OR
- New horizontal or descending ST depression ≥ 0.05mV and / or T inversion ≥ 0.1mV in two contiguous leads with proeminent R wave or R/S ratio> 1.
